ASSISTED LIVING CHECKLIST

PROTECTION AND PLANNING

  • Is there a written plan for the care needs of each resident?
  • Does the family and physician(s) participate in the resident’s written care needs plan?
  • Is there a written Agreement Contract on file for every resident?
  • Does the Agreement Contract disclose all policies and fees for
    • Health care and supportive services?
    • Admission and discharge provisions?
    • Refunds and transfers?
  • Is there a process to periodically review the residents written care needs plan?
  • Is the name of the local Ombudsman conspicuously posted and visible throughout?

PHYSICAL FACILITIES

  • Is the floor plan easy to follow?
  • Are doors, halls and rooms accommodating to wheelchairs and walkers?
  • Are handrails and non-slip floors evident and convenient?
  • Is the facility free of offensive odors and HVAC regulated with adequate lighting?
  • Does the facility have fire/smoke protection devices and sprinklers and clearly marked exits?

MEDICATION AND HEALTH CARE

  • Does the facility have specific policies on medication for record keeping, storage and staff training?
  • Is there a staff person that coordinates home health care visits?
  • Is staff available to assist residents who experience orientation or judgment losses?
  • May you use your own physician?
  • Is there a clearly stated policy for responding to a resident’s medical emergency?

FOR NURSING HOMES ONLY

  • Are fraud and abuse posters posted and visible throughout?
  • Is the staffing ratio of patients to residents posted and visible throughout?
  • Is the quality of care ratings posted and visible throughout?
  • Were the administrator and staff visible, involved and easily identified?
